This waiver must be filled out completely and signed by each player that is competing in the Tournament and must be signed by the Parent or Legal Guardian of that player.

If you wish to fax your registrations form you may fax them to 607-697-1878 On the cover sheet please put them name and phone number of your school and the number of pages being sent to include the fax cover sheet.
